At Holistick IV LLC, we value our clients’ time and strive to provide the highest level of service. To ensure smooth scheduling and accommodate all clients efficiently, we have established the following Cancellation & Refund Policy in compliance with Florida state regulations.
Cancellation & Rescheduling Policy
Cancellations with 3+ Hours’ Notice
- Clients may cancel or reschedule their appointments at least 3 hours before the scheduled time without any penalty.
- Cancellations or reschedules can be made via our online booking system or by contacting our office at

Cancellations/Rescheduling with 30 Minutes to 3 Hours’ Notice
- Any changes or cancellations made between 30 minutes and 3 hours before the scheduled appointment may be subject to a $50 cancellation/rescheduling fee.
- This fee helps compensate for the reserved time and operational costs incurred due to last-minute changes.
No-Shows & Last-Minute Cancellations
- If a client fails to show up, arrives more than 15 minutes late, or cancels/reschedules within 30 minutes of the appointment time, a $100 cancellation/rescheduling fee may be charged.
- Repeated no-shows or last-minute cancellations may result in denial of future appointments or require prepayment for future bookings.
Refund Policy
General Refund Guidelines
- All refund requests must be approved by Holistick IV LLC. Once approved, refunds will be processed within 10 business days.
- Refunds will be issued to the original payment method (credit card or other applicable method). Processing times may vary depending on the client’s financial institution.
Refunds for Services
- Refunds for completed services are generally not provided, except in cases of:
- Medical necessity, such as an adverse reaction or provider-determined contraindication.
- Service-related issues, including procedural errors or provider inability to administer the requested treatment.
- The $49 Good Faith Exam provided by our advanced practitioner via telehealth, that is required prior to the client’s first visit and as determined by the practitioner, is non-refundable.
- If a refund is issued for a completed service, the amount refunded will be at the discretion of Holistick IV LLC based on the circumstances.
Refunds for Prepaid Services & Packages
- Clients who prepay for a package or membership plan but choose to discontinue services may request a pro-rated refund, provided no treatments have been used.
- If any treatments have been redeemed, refunds will be issued at the standard single-session rate, and any remaining balance will be refunded accordingly.
- Membership fees are non-refundable once services have been used unless cancellation is due to a documented medical reason.
- Prepaid membership fees are refundable if the member has not used any services during the billing period, subject to the terms of the Membership Subscription Agreement.
Additional Terms & Conditions
- By booking an appointment, clients acknowledge and agree to the terms outlined in this policy.
- Holistick IV LLC reserves the right to modify this policy at any time to reflect changes in operations or legal requirements. Any updates will be posted on our website [www.holistickiv.com].
- Clients disputing charges should contact us directly before initiating a chargeback with their financial institution. Chargebacks may result in legal action and account restrictions.
- Emergencies & Special Circumstances: If a client is unable to attend their appointment due to an emergency or extenuating circumstance, we may waive or modify cancellation fees at our discretion. Supporting documentation may be required.
